* This position is unique for offering a Baylor shift. This means you will work two 12 hour shifts, but get paid for 36 hours.
**Title:** Hospice Visit RN \\- Baylor Shift
**Schedule:** 6pm to 6am, Fridays \\& Saturdays. Work 24, paid for 36\\.
**Location:** In the field, visiting patient homes. Area spans near Loveland area in the north and Parker area toward the south.
**Pay:** $42\\.34 to $50\\.81 per hour
ESSENTIAL DUTIES AND RESPONSIBILITIES
* The Afterhours Registered Nurse will primarily conduct afterhours home visits and provide care to patients, as delegated by the Phone Triage Nurse.
* Provides nursing care to meet the patientβs needs in accordance with the patientβs hospice plan of care.
* Cross\\-train on phone triage to assist Triage RN team as necessary.
* Correctly identifies, communicates, and collaborates on patient problems/priorities/issues based upon assessment data. Establishes measurable patient goals and communicates an on\\-going plan of care appropriately to the interdisciplinary team (IDT). Recognizes changes in the clinical status of the patient and reports findings to the appropriate team member.
* Collaborates with patient/family when developing the plan of care, adjusting communication style as necessary.
EDUCATION \\& REQUIREMENTS
* Must possess a current RN license in the state of Colorado
* Must have a minimum of one year RN experience β hospice or home health experience highly preferred
* Must successfully pass required background checks and drug screening
* Requires travel to multiple work locations
