**Title:** Hospice Phone Triage RN
**Pay:** $42\\.34 to $50\\.81 per hour
**Schedule:** Saturday/Sunday from 7pm\\-7am
**Location:** Mostly remote within Colorado. Will require occasional training near, and hiring preference for, Lafayette, CO
JOB SUMMARY
TRU Community Care is looking for a phone triage nurse. This nurse will work with our afterhours and triage team, taking in calls from patients and collaborating with visit nurses on patient needs as necessary.
ESSENTIAL DUTIES
* The Triage Registered Nurse performs in a supportive role to the Care Team by answering calls coming into the agency, with response to the telephone call by delegating tasks.
* Provides nursing care to meet the patient’s needs in accordance with the patient’s plan of care.
* Correctly identifies, communicates, and collaborates on patient problems/priorities/issues based upon assessment data. Establishes measurable patient goals and communicates an on\\-going plan of care appropriately to the interdisciplinary team (IDT). Recognizes changes in the clinical status of the patient and reports findings to the appropriate team member.
* Collaborates with patient/family when developing the plan of care, adjusting communication style as necessary.
EDUCATION \\& REQUIREMENTS
* Must possess a current RN license in the state of Colorado
* Must have a minimum of one year RN experience.
* Hospice or home health experience highly preferred.
* Hospice triage experience preferred.
* Must successfully pass required background checks and drug screening
