**What We Are Looking For:**

The Admissions Nurse is responsible for overseeing the admission and readmission process for residents entering the facility. This includes completing comprehensive nursing assessments, ensuring accurate and timely documentation, coordinating with the interdisciplinary team, and communicating effectively with residents, families, and outside providers to ensure a smooth transition into the facility.
**What You Will Do:**

**Resident Admission \\& Readmission**
* Complete all required nursing admission assessments and documentation in accordance with facility, state, and federal guidelines.
* Ensure accurate medication reconciliation and review all physician orders upon admission or readmission.
* Develop initial care plans in collaboration with the interdisciplinary team.
* Obtain and verify all necessary documentation from hospitals, physicians, and other healthcare providers.
* Coordinate the transfer of resident belongings, equipment, and personal needs into the facility.
**Coordination \\& Communication**
* Serve as the primary nursing contact for new admissions and their families, providing orientation to the facility.
* Communicate effectively with hospital discharge planners, physicians, and other referral sources to obtain necessary clinical and personal information.
* Notify appropriate departments of incoming admissions and provide relevant clinical details to ensure readiness.
* Work closely with the MDS/Care Plan Coordinator to ensure accurate assessments and care plan development.
**Compliance \\& Quality Assurance**
* Ensure all admission documentation is completed accurately and within required timelines.
* Follow HIPAA guidelines and maintain resident confidentiality.
* Participate in quality improvement initiatives related to the admissions process.
* Maintain compliance with all state and federal regulations governing skilled nursing facility admissions.
**What You Will Bring:**

* Current, valid Illinois RN license in good standing.
* Minimum of 1 year nursing experience; long\\-term care or skilled nursing facility experience preferred.
* Strong assessment, organizational, and communication skills.
* Ability to work efficiently in a fast\\-paced environment and manage multiple admissions.
* Proficiency in electronic medical records (EMR) systems preferred.
**Physical Requirements:**

* Ability to lift, push, pull up to 50 lbs and assist with resident transfers when needed.
* Frequent standing, walking, bending, and reaching in a healthcare environment.
