**Who We're Seeking:** Join our team as a Per Diem Weekend Home Health RN to work with patients in their home environments.
**Your Responsibilities:** As a member of our dedicated team, you'll deliver high quality care directly to individuals within the comfort of their homes in a designated geographic territory. You will collaborate with colleagues, local physicians, and community providers to provide exceptional health care to our clients.

**Requirements:**
* VT Licensed RN (BSN preferred)
* Home care experience and/or previous case management experience in a home care setting and discharge planning and/or utilization review experience.
* Valid driver’s license
