**Job Description:**
* PURPOSE OF JOB: Assist with all aspects of delivery of care and support for patients in an outpatient setting including:
+ Assisting physicians and extenders (NP, PA's)
+ Patient assessments and monitoring
+ Patient education
+ Triage nursing (telephonic and in person)
+ Coumadin management
+ Operation of equipment
+ Chronic care management
+ Risk stratifying of patients to identify high risk/ high cost patients and their medical needs
+ Telephonic outreach to patients that have been discharged from the hospital, nursing home, emergency room and/or urgent care
+ Other duties as requested
* ESSENTIAL FUNCTIONS:
* Manages the day to day support of patient care with patient assessments, plans, education, implementation, and documentation, coordination of care according to department and hospital standards. Utilize hospital and office based electronic medical records system to obtain and document care, including both clinical support for office visit, electronic communication, and telephone communication
* Communicates and collaborates with patients, physicians, family members and other health care disciplines in order to achieve excellence in patient care. Maintains competency in communicating electronically. Including but not limited to the management of lab results, telephone triage (screen for urgency and level of care required). Receive calls from Visiting Nurses and other allied health professionals prioritize and communicate with care team as needed.
* Promotes patient safety through adherence to protocols, policies, procedures, evidence\\-based guidelines and OSHA Goals.
* Actively participates in and supports departmental and organizational quality initiatives and goals. Understands and follows office policies and procedures.
* Maintains clinical competency according to unit\\-specific standards and licensure guidelines.
**Job Qualifications:**
QUALIFICATIONS:
* Education:
- Graduate of an accredited School of Nursing. RN required.
- Current licensure in Commonwealth of Pennsylvania, Department of State Bureau of Occupational and Professional Affairs and successful completion of appropriate continuing education specific to job requirements
* Experience: Cardiology experience preferred
* Other Skills:
* Certified in BLS for Healthcare Provider
* Excellent interpersonal and team building skills
* eClinical Works experience preferred
* Knowledge of the following:
+ State law requirements
+ Certification requirements
+ CME requirements
+ Chart/medical record documentation
+ Federal/state laws related to the release of health care information
+ Computer functionality
+ Nursing processes; health care system, structure and functions
* Skill in triaging patients, in\\-person and by phone
* Ability to calculate and administer drug dosages/injections and measure results
* Skill in initiating appropriate crisis interventions and emergency response
