**The role you'll contribute:**
The Stroke Program Coordinator, in collaboration with stroke medical director and site and regional clinical leadership, are responsible for the coordination of clinical excellence in stroke care across the continuum of their respective AdventHealth site. At one assigned campus, the coordinator functions as a member of the interdisciplinary team, responsible for monitoring and improving stroke program performance as well as national accreditation and certification requirements. The coordinator facilitates the development and implementation of stroke\\-related educational initiatives for staff, patients, community and emergency medical services. The coordinator leads their campus interdisciplinary teams in performance improvement and the incorporation of evidence\\-based practice protocols into the daily operations of the stroke program. The coordinator is responsible for analyzing outcomes to determine root causes of variation and developing corrective actions to improve stroke outcomes and performance. The coordinator works with community and professional organizations to further community education and awareness about stroke prevention and treatment. The coordinator actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

**The value you'll bring to the team:**
* Serves as the stroke program clinical content expert, utilizing skilled knowledge of clinical stroke evidence\\-based guidelines, national accreditation standards, and regulatory requirements.
* Leads the stroke interdisciplinary committee(s) in partnership with the stroke medical director to improve clinical performance, standardize processes, and ensure compliance with accreditation and regulatory standards.
* Identifies variations in care and leads the development of corrective action plans to improve performance.
* Leads mortality and morbidity committee reviews to identify clinical opportunities for improving outcomes.
* Manages the initial and ongoing national accreditation process, including readiness rounds, mock stroke alerts, education, and documentation to meet accreditation standards.
* Coordinates stroke educational plans and communicates updates on outcomes, process changes, stroke awareness, and regulatory requirements across campuses and EMS agencies.
* Organizes community education events to raise stroke awareness and promote prevention strategies.
* Rounds on stroke patients to provide additional education to patients, families, and staff, and participates in multi\\-disciplinary rounds to ensure safe and timely discharge.
* Responds to and participates in stroke alert events to support the clinical team and completes follow\\-up calls to determine patient outcomes and provide additional education and support.
* Performs other duties as assigned.
* Develops and reviews evidence\\-based clinical pathways to standardize stroke care across the continuum.
* Provides real\\-time feedback to campuses on stroke patients and processes to drive performance improvement.
* Evaluates stroke program metric performance, ensuring accurate reporting of process and registry measures, and shares outcomes with committees and campus leadership.
Qualifications
**The expertise and experience you'll need to succeed** **:**
* Bachelor's of Nursing Required
* Master's Preferred
* Three or more years of Work Experience Required
* Up to date knowledge of adult neuroscience and stroke nursing practice
* Strong leadership, organization, and collaboration skills
* Excellent analytical and problem\\-solving skills
* Knowledge of process improvement strategies
* Effective oral and written communication skills with the ability to articulate complex information in understandable terms to all levels of staff
* Ability to manage multiple competing priorities to achieve organizational goals
* Effective computer skills, particularly Microsoft Office (Excel, Word, and PowerPoint)
* Knowledge related to telemedicine practice
* Knowledge of American Heart Association/American Stroke Association Get with the Guidelines Stroke Registry
* Knowledge of state/federal regulatory agencies, accreditation standards and national society clinical guidelines
* Quality improvement knowledge include but not limited to Centers for Medicare \\& Medicaid Services (CMS) outcomes, Value Based Purchasing, Bundle Payment, and other national quality improvement initiatives
* Registered Nurse (RN) Required
* Basic Life Support \\- CPR Cert (BLS) Required
* RN Other Certification (RN) Neuroscience Nursing (CNRN), Critical Care Nursing (CCRN), Stroke Certified Registered Nurse (SCRN) Preferred
* Advanced Cardiac Life Support Cert (ACLS) Preferred
* NIH Stroke Scale Certification (NIHSS) Preferred
* Change Management Certification (PROSCI) Preferred or
* Six Sigma (SS) Preferred or
* Lean Bronze Certification Preferred
