**Job Summary**
The Behavioral Health nurse works with the interdisciplinary Behavioral Health team to ensure that patients with behavioral health concerns receive symptom\\-directed care that is within established standards of care.

Primary Duties:* Provides clinical support to provider(s) and staff in the Behavioral Health clinic.
* Serves as a liaison to ensure continuity of care among the Behavioral Health clinic and primary care providers, hospitals, EDs, and specialists.
* Triages patients via MyChart messages and phone.
* Works with healthcare providers to develop and implement each patient’s plan of care.
* Assists in answering symptom management and medication questions and concerns in MyChart.
* Maintains and analyzes program metrics in collaboration with team leaders.
* Provides back\\-up coverage for the Clinic Coordinator
* Assists with prior authorizations for medications.
* Provides behavioral health resource referrals as appropriate

**License, Certification, Education or Experience:**



REQUIRED for the position:* Graduate of a nursing program (Diploma or Associate Degree in Nursing (ADN) required)
* Current BLS Healthcare Provider certification by date of hire.
* 2 years of Home Health, Hospice or other Home Care experience.
* Must have prior marketing, sales and/or community outreach experience.
* Current Washington State Driver’s license and proof of insurability, if the position requires regular driving.
* Must have working knowledge of Medicare COP, JC Standards
* Must be familiar with Third party insurance and Medicare HMO’s.
