📝 Description: **Horizon Health** is a Critical Access, Rural Health Facility comprised of 25\\-inpatient beds located in Paris, IL \\& a multitude of outpatient clinic settings including Family Practice and Specialty Clinics in Paris and surrounding cities. We have been serving residents of Edgar County since 1968 though community education, emergency services, and outpatient care. **Position Summary:**


Acts as a patient advocate/navigator to facilitate care through interdisciplinary collaboration and care coordination with interdisciplinary staff both internally and on an external basis as needed. Provides care coordination to facilitate the delivery of cost\\-effective quality healthcare and assists in the identification of appropriate utilization of resources across the continuum of care in an autonomous role.


Actively participates in quality improvement and evaluation processes related to the navigation and management of patient care coordination.


Responsible for communicating with patients and caregivers to ensure improved care transition, enhancing patient\\-practitioner communication to identify opportunities for care continuity, encouraging the patient’s self\\-management skills and understanding of the health care system to guide and empower patients to participate actively in their care plan.

**Essential Functions (Responsibilities/Accountabilities):**

* Educate patients and their families about chronic illness and processes to reduce readmissions.
* Collaborate with practitioner/care team to identify and engage patients to participate in care coordination activities.
* Ensure that a care plan is in place with patient and care team and assist patient in goal setting for self\\-management and monitor adherence to defined care plan and appropriate utilization of healthcare services.
* Emphasize continuity of care, thereby reducing or eliminating fragmentation, duplication, and gaps in treatment plan.
* Act as patient advocate, protecting privacy and confidentiality issues
* Coordination of education, monitoring of health needs and community resources.
* Prevent adverse patient occurrences when possible and intervene quickly if prevention is not possible, thereby minimizing poor outcomes.
* Facilitate patient empowerment and quality of life by promoting educated, independent patient choices on all aspects of care.
* Maintain a comprehensive working knowledge of community resources, payer requirements, and
* network services for target population.
* Identify opportunities for health promotion and illness prevention,
* Present current practice case studies at educational sessions.
* Serves as a patient advocate, capable of problem solving in a patient centric manner.
* Maintain knowledge base of internal and external regulations as well as available services
* Collaborate with hospital and clinic navigators, social workers and case managers
* Maintains proficiency of RN license, Electronic Health Record and required certifications
* Attend meetings as required.
* Performs all other duties as assigned.

**Position Requirements:**

* Current State of Illinois RN license with BSN preferred
* Current ACLS and PALS certification
* Trauma experience preferred
* At least five years of acute care hospital experience.
* Home Health experience desirable.
* Demonstrated skills in the areas of communication (verbal and written), conflict resolution, interdisciplinary collaboration, management, creative problem solving, and critical thinking.
* Knowledge of post\\-acute care community resources.
* Case management and or care coordination experience preferred.
* Chronic disease management experience.

**Pay Range:**


Pay ranges from $27\\.473/hour to $43\\.997/hour (rate of pay is based on applicable years of experience)

**Hours: Monday\\- Friday 4 10hr shifts, typically 8a\\-6p**
