We’re seeking a Registered Nurse (RN) – Clinical Manager to oversee clinical operations and patient care services in our home health division. This is your opportunity to lead with compassion, drive excellence, and be part of a team that treats each other like family.


Key Responsibilities:* Provide daily clinical oversight of patient care services.
* Coordinate referrals and admissions with internal and external sources.
* Evaluate patient needs and agency capabilities before admission.
* Supervise care planning, partner assignments, and care delivery.
* Ensure compliance with regulatory standards and agency policies.
* Mentor and coach clinical staff to promote morale and retention.
* Utilize MatrixCare and Strategic Healthcare Partner systems for workflow and reporting.

Qualifications:
Required:

* Active, unrestricted RN license in the state of practice.
* Minimum of two years RN experience (preferred)
* At least one year of Home Health experience (preferred)
* Current CPR certification.
