Cheer Home Care is hiring a RN Care Case Manager in San Diego. The RN Care Case Manager collaborates with interdisciplinary team members to develop and implement individualized care plans and provides constant feedback to clients, client representatives, and nurses. They work closely with our office team and have excellent communication and problem\\-solving skills.
