#### **Duties**
Conduct interdisciplinary team meetings to determine appropriate services
Provide accurate, timely, complete, and acceptable documentation
#### **Write State Plans**
Monitor patient progress and respond to changes in condition.
Assist in family planning consultations and education.
Assist with the transition of the individual from services, if necessary, and planning for any future needs
Collaborate with healthcare professionals to develop and implement individualized care plans.
Provide education and support to patients and their families regarding health management.
Maintain accurate patient records and documentation in compliance with healthcare regulations.
Participate in quality improvement initiatives and adhere to safety protocols.
Experience
#### **Candidates should have:**
A valid Registered Nurse (RN) license.
Experience in Home Health care is preferred.
Strong interpersonal skills for effective communication with patients, families, and team members.
A commitment to ongoing professional development and staying current with nursing practices.
