### **Position Summary**



The Supervisor of Clinical Services will oversee and coordinate the daily clinical operations and care delivery teams for Affinity Hospice of Connecticut. This role ensures that our hospice patients and their families receive compassionate, highquality care in accordance with our mission and regulatory standards. The Supervisor supports clinical staff (RNs, LPNs/LVNs, hospice aides, social workers, bereavement staff) in implementing interdisciplinary care plans, managing staffing and scheduling, monitoring quality and compliance, and fostering a culture of teamwork and continuous improvement.



### **Key Responsibilities**


* Supervise, mentor, and support clinical staff across home care, assisted living, and skilled nursing facility settings.
* Ensure timely assessments, plan of care development, and interdisciplinary team (IDT) meetings for hospice patients.
* Monitor and ensure compliance with state and federal regulations (including Medicare hospice benefit requirements, documentation, licensure) as well as internal policies and accreditation standards.
* Oversee staff scheduling, oncall rotation, visit coverage, and staffing models to maintain patient care continuity.
* Collaborate with the admissions/referral team to ensure smooth clinical onboarding of new patients and transitions of care.
* Review clinical documentation, patient outcomes, and quality metrics; identify and implement process improvements.
* Provide leadership in symptom management, endoflife care best practices, patient/family education, and staff education/training.
* Act as a clinical resource for staff, patients and families — supporting complex clinical cases, ethical issues, and family communication.
* Participate in budgeting and resource planning for clinical operations as needed; monitor supply usage and equipment needs.

### **Qualifications**


**Education \\& Licensure:**


* Registered Nurse (RN) with active Connecticut license.
* Bachelor’s Degree in Nursing (BSN) preferred; relevant master’s degree or leadership preparation a plus.

**Experience:**


* Minimum of 35 years of hospice or palliative care nursing experience (home care or facility\\-based) required.
* Prior supervisory or leadership experience in a clinical setting preferred.
* Familiarity with hospice regulatory requirements, clinical documentation, IDT processes, and interdisciplinary care.

* Strong clinical judgment, problemsolving skills, and comfort with endoflife care and hospice philosophy.
* Excellent organizational skills, attention to detail, and competency in electronic medical records and documentation systems.
* Commitment to maintaining clinical excellence, quality improvement, patientcentered care, and regulatory compliance.
