**Job Summary**
We are seeking a **Registered Nurse** to join our team. In this role, you will primarily be creating and managing a plan of care for individual clients with intellectual/or developmental disabilities (IDD). We provide services that assist an individual in gaining access to needed medical, educational, and/or other services to include:
HCS \\- Home and Community\\-Based Services, Provider Care, Texas Home Living, Independent Home Living, and Notary Services.
Our Case management services may be used to locate, coordinate, and monitor necessary appropriate services. It may be used to encourage the use of cost\\-effective medical care by referrals to appropriate providers and to discourage the over\\-utilization of costly
services.
**Responsibilities**
* Completes a CNA (Comprehensive Nursing Assessment) upon admission to the non\\-residential program and annually thereafter.
* Completes quarterly (or semi\\-annual) nursing assessments.
* Coordinates the health services of clients on assigned caseload.
* Ensures face\\-to\\-face visits with clients occur as developed in the individual plan.
* Provides training to clients, families, HH/CC guardians as necessary related to client health care needs.
* Follow up with medical needs that the client or their guardian, HH/CC provider may identify or ask for assistance.
* May assist with obtaining AA (Adaptive Aids) or DME (Durable Medical Equipment).
* May be required to complete Title XIX documentation.
* Available to communicate with clients, families or guardians to answer questions or provide information.
* May assist and facilitate physician appointments for clients and/or providers.
**Qualifications**
* Currently licensed Registered Nurse (RN) in Texas
* Experience with waiver services and HHS regulations as related to IDD nurses preferred
* Previous experience as a Registered Nurse is preferred
* Excellent interpersonal and client care skills
* Valid driver’s license
* First aid/CPR certification required
