**POSITION SUMMARY**
The Registered Nurse After Hours (RN) in accordance with public health nursing standards, practices, and policies, will provide and arrange for Hospice care for patients/families, in a collaborative relationship, with other members of the interdisciplinary team. The Registered Nurse After Hours (RN) makes initial nursing assessments, judgments and treatment decisions based on patient/family needs and wishes in accordance with professional skills.

**ESSENTIAL DUTIES AND RESPONSIBILITIES**
**Emergency Response and Patient Visits:**
* Answer emergent patient calls and conduct necessary visits to address immediate concerns.
**Nursing Care:**
* Provide professional nursing care in line with the established patient care goals, ensuring all actions are aligned with the physician’s plan of care.
* Assess and evaluate patient status; if working as an LPN, report findings to the RN on duty.
**Care Planning and Implementation:**
* Develop and update care plans that establish goals based on nursing diagnoses and incorporate palliative nursing actions.
* Initiate appropriate preventive and rehabilitative nursing procedures, administering medications and treatments as prescribed.
**Patient and Family Counseling:**
* Counsel patients and families on nursing and related needs, providing health care instructions as per the assessment plan.
* Teach self\\-care techniques to patients and caregivers, including medication, diet, and health counseling opportunities.
**Documentation and Communication:**
* Complete, maintain, and submit accurate and relevant clinical documentation regarding the patient’s condition and care provided, including pain and symptom management.
* Communicate with physicians regarding patient needs and any changes in condition, obtaining and documenting physician orders as required.
**Environment and Equipment Management:**
* Provide and maintain a safe environment for the patient.
* Coordinate arrangements for necessary equipment and services.
**Continuity of Care:**
* Conduct initial assessments of patients and families, ensuring continuity of care by collaborating with the patient, family, and other team members.
**Professional Development:**
* Assume responsibility for personal growth and professional development by attending continuing education and in\\-service classes.
**Quality Improvement:**
* Actively participate in quality assessment and performance improvement initiatives.
* Minimize non\\-productive time by engaging in activities like peer chart reviews and medication reconciliation checks.
**Community and Facility Outreach:**
* Contact facilities within the service area to identify and address patient needs.
* Assist with the tuck\\-in program and other patient support services as assigned.
**EDUCATION AND EXPERIENCE**
* RN license in the Commonwealth of Virginia
* Graduated from an accredited nursing school. BSN preferred
* 2\\+ years of experience in hospice, home health, or ICU CCU, preferred
* CHPN certification, preferred
* Basic knowledge of Medicare/Medicaid reimbursement regulations
* Current CPR Certification
* Strong communication and interpersonal skills
* Proficient computer skills and electronic medical records required.
* Must be able to travel throughout the territory as needed.
**PHYSICAL/SENSORY/COGNITIVE REQUIREMENTS**

* Must have visual acuity enough to continuously note changes in patient condition (breathing, color, and skin integrity) and to read a thermometer, sphygmomanometer dial, and other instruments.
* Must have hearing acuity enough to auscultate BP, hear verbal instructions and communication from clients, respond to emergency alarms, and answer the telephone.
* Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, and/or crouching. Visual acuity and hearing to perform required nursing skills
* Must be able to lift to 50 pounds.
