### **Position Summary:**
We are seeking a compassionate and skilled **Registered Nurse (RN)** to join our HomeCare team in **Baltimore County, MD**. This position involves providing in\\-home nursing care to patients, ensuring high\\-quality, family\\-centered services. As a key member of our interdisciplinary care team, you will conduct patient assessments, develop individualized care plans, and collaborate closely with patients, families, and physicians to promote positive health outcomes.
### **Essential Functions:**
* Perform initial and ongoing patient assessments in the home setting.
* Complete thorough and accurate **OASIS** documentation.
* Develop and implement individualized care plans based on patient and family needs.
* Deliver professional, safe, and effective nursing care according to established standards.
* Educate patients and caregivers throughout the care process and during discharge planning.
* Coordinate with community resources and other healthcare professionals to optimize care.
* Supervise and provide guidance to **Licensed Practical Nurses (LPNs)** and **Home Health Aides**.
* Document patient progress and revise care plans as needed to achieve desired outcomes.
### **Qualifications:**
* **Current Registered Nurse (RN) licensure** in the State of Maryland.
* **American Heart Association BLS CPR** certification (for healthcare providers).
* **Valid driver’s license in good standing** and reliable transportation.
* **Graduate of an accredited School of Nursing**.
* Minimum of **1 year of medical\\-surgical (med\\-surg) experience** preferred.
* Strong clinical assessment and critical thinking skills.
* Experience with OASIS documentation a plus.
