A Public/Community Nurse is a highly skilled healthcare professional who plays a vital role in improving the health and well\\-being of individuals, families, and communities. This role involves providing a wide range of healthcare services in community settings, addressing the unique healthcare needs of diverse populations. Public/Community Nurses focus on health promotion, disease prevention, and delivering patient\\-centered care to support the overall health of their communities.

**Examples of Work Performed:**


Conduct comprehensive health assessments of individuals and communities to identify healthcare needs, risks, and priorities.


Develop individualized care plans based on assessment findings, addressing physical, emotional, and social aspects of health.


Provide health education to individuals and groups on preventive care, healthy lifestyle choices, and disease management.


Organize and participate in community health promotion activities, workshops, and outreach events.


Administer immunizations and vaccinations to prevent the spread of communicable diseases.


Perform health screenings for conditions such as hypertension, diabetes, and sexually transmitted infections (STIs).


Deliver skilled nursing services, including wound care, medication management, and chronic condition monitoring for homebound individuals.


Collaborate with healthcare teams to ensure coordinated care for patients with complex or ongoing medical needs.


Establish and maintain strong relationships with community members, leaders, and organizations to identify and address healthcare disparities.


Advocate for community health needs and actively participate in local health committees, coalitions, and initiatives.


Support disaster preparedness and emergency response efforts, including providing immediate care and support during crises.


Maintain accurate, timely documentation in the IHS Electronic Health Records system, including patient records and community health data.


Compile and submit required reports related to public health trends, outcomes, and intervention efforts.


Address cultural, social, and economic factors that contribute to health disparities, ensuring culturally competent care.


Engage in ongoing professional development to remain current with healthcare best practices, trends, and regulatory changes.


Participate in quality improvement initiatives to enhance healthcare delivery and outcomes.


Provide in\\-home nursing assessments and wound care as needed.


Receive, coordinate, and disseminate updated health information, medication changes, and provider orders from external agencies and IHS to the appropriate health professionals.


Assist patients in coordinating care with Tribal Health, Home Health programs, and local community resources.


Support monthly nail care clinics in collaboration with the Home Health team.


Manage care for MOUD/MAUD patients, including discharge planning and patient education starting at time of booking.


Educate patients on proper medication administration and distribute daily medications in accordance with provider orders.


Perform controlled substance reconciliation and medication counts to ensure compliance and safety.


Conduct timely data entry and assist with monthly, quarterly, and annual reporting requirements.


Work collaboratively with interdisciplinary teams, including Tribal Jail staff and prescribers.


Train and support medical assistants to ensure clinical competency and program consistency.


Perform other duties as assigned to support overall program goals and community health.

**Knowledge, Skills and Abilities:**


Knowledge of and the ability to apply nursing principles, practices, and procedures required to assess patient


needs in all aspects of health as well as knowledge of pathophysiology, anticipated complications and indicated therapeutic interventions.


Relevant experience in community or public health nursing.


Strong interpersonal and communication skills.


Knowledge of public health principles, epidemiology, and healthcare policies.


Cultural competence and the ability to work with diverse populations.


Proficiency in electronic health records and healthcare software.


Knowledge and skill in operation of specialized medical equipment.


Ability to work independently in both clinical and public health nursing practice. This includes the ability to work on own initiative with minimal supervision, prioritizing and completing projects, assignments, and work duties in a timely manner and being innovative in creating new systems for optimal patient care.


Ability to receive medication orders and changes accurately from health care providers.

**Minimum Requirements:**


Registered Nurse (RN) licensure and Bachelor of Science in Nursing (BSN) required.


Current Active RN, BSN State of Washington license with no record of disciplinary actions.


Certification in public health nursing or related field is an asset.


Up to date on CEU's per state requirements.


Up to date on first aid/CPR certification.


Must pass Indian Health Services background check.


Required to pass a pre\\-employment background check.


Required to pass a pre\\-employment drug test.


Must possess a valid Washington State Driver's License with the ability to obtain a Yakama Nation Driving permit.
