📝 Description: We are currently looking for a Full Time Referral Intake LPN to join our Home Health Clinical team located at the Chickasaw Nation Medical Center working 8:00AM\\-5:00PM \\- Mo Tu We Th Fr.


**Summary:**



Responsible for managing all aspects of the patient intake process including, scheduling, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns.


**Responsibilities:**

* Reports on patient’s condition and significant changes to the assigned case manager
* Participates as a member of the interdisciplinary team
* Adheres to Minticha Apila Home Health’s documentation and care procedures and standards of personal and professional conduct
* Directs all patient referral and intake operations including scheduling of patients into the electronic health record timely
* Receives and transcribes verbal and written orders from providers
* Ensures compliance with all state, federal, and Joint Commission referral/intake requirements
* Follows the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy
* Establishes and maintains a positive working relationship with current and potential referral sources
* Ensures seamless transition of patients to home care by providing effective communication with patient and or family members in preparation for home care, including when the plan of care initiation will be, and coordination of care with multiple service providers
* Ensures timely input of referrals, orders and schedules throughout the workday and ensures referral documents are placed into the patient’s electronic health record
* Appropriately follows up ensuring provider orders are transmitted, signed, received back and placed into patients’ electronic health record for timely submission and billing purposes
* Updates and informs case managers regarding start of care orders, verbal orders or other correspondence that occurs while clinicians are in the field
* Builds and supports community and customer perceptions of the Chickasaw Nation as a high\\-quality provider of home health services
* Gathers, collates, and reports statistics including missed visit reports, falls, variances and obstacles including staff inconsistency with their schedules to the Director of home health
* Answers phone calls and greeting visitors to the home health office as needed
* Provides first class customer service \\- courteous and helpful to all internal and external customer types
* Answers questions in a courteous and professional manner
* Greets and engages with a smile
* Determines needs, seeks positive experience and outcomes
* Completes all training and development requirements and participates in continuing education opportunities
* Works in a collaborative and supportive manner to support the productivity and safety of the work area, team members and customers of all types
* Maintains general upkeep of work area and maintains a safe environment
* Maintains and promotes professionalism
* Punctual and consistent in meeting start time, work schedule, attendance and other assigned work time metrics
* Adheres to Chickasaw Nation dress code standards
* Maintains knowledge of and follows all laws, rules, regulations, policies and SOPs for assigned area, e.g. MICS, TICS, hospital standards, health codes, food sanitation, accounting principles, etc.
