3 night shifts per week
Every 3rd Holiday
Every 3rd Weekend
12 Hours of On-Call Every 3 Weeks
Scheduled Hours: Full Time; Night Shift 7pm to 7am - including every 3rd weekend, every 3rd holiday and 12 hours of On-Call every 3 weeks.
Licensure, Registration, and/or Certification Required:
CPR certified
CMA or CNA or LPN or Paramedic or CST
Education Required:
High School diploma or equivalent
Experience Required:
Certified Nursing Assistant or Nursing Student with first clinical completed or Certified Medical Assistant or Paramedic or LPN or Certified Scrub Tech.
1 year experience preferred.
Knowledge, Skills & Abilities Required:
Knowledge of basic computer keyboard functions is preferred. -Ability to communicate both verbally and in writing. -Courteous and professional interpersonal skills. -Demonstrates responsible and ethical behaviors consistent with the Advocate values.
Major Responsibilities:
Provides general patient care duties that are specific to the patient population served, under the direction by the Team Leader
1)Provides personal care to include bathing the patient, oral and denture care, shave, backrub and skin care.
2)Assists patient with appropriate activity order, including proper positioning, ambulatory, ROM and special transferring needs.
3)Assists with toileting needs of patients including toileting cues, care of the incontinent patient, proper use of diapers, identification of patients with high risk for skin breakdown
4)Responds to call lights and patient requests or refers to appropriate person
5)Provides care and maintenance of indwelling catheters or tubes
6)Collects, prepares, and transports as directed by Team Leader
7)Assist physicians with bedside procedures as delegated by the Registered Nurse.
8)Perform oral suctioning using a yankauer suctioning instrument, as delegated by RN
9)Performs scrub functions during surgical procedures.
10)Hourly round
Participates in the Admission/Discharge/Transfer process of all patients under the guidance of the Registered Nurse, or designee
1)Sets up patient's room
2)Orients patient and family to room, including physical layout of the room, call lights, bed functions, TV use, and telephone
3)Obtains vital signs including height and weight
4)During admission/transfer/discharge ensures that all patient's belongings are accounted for and documented.
5)Provides post-mortem care.
6)Maintains and updates as necessary and prints the daily census as well as nurse report sheets from the computer.
7)Photocopies any necessary records for transport to another facility
8)Communicates with appropriate departments the information related to Admission/Discharge/Transfer.
9)Connect patient to external fetal monitor as instructed by RN
Perform tasks related to meeting the nutritional needs of the patient.
5)Monitors intake and output and provide accurate information for calorie count.
1)Reviews diet order to ensure patient is receiving proper diet
2)Assists patient with meals, and infants with feedings, including passing water and nourishments
3)Provides proper tray setup and patient positioning with consideration to special needs
4)Assists patient in menu selection.
Contribute to the patient plan of care through the documentation and/or communication of patient needs, problems and outcomes to other health care providers
1)Documents patient information on appropriate forms
2)Listens and responds to internal/external customers in a respectful manner
3)Reinforces specific patient and family teaching under the direction and supervision of the registered nurser to insure patient access to appropriate information and resources
4)Acts as a patient/family advocate by providing comfort, assistance and information.
Perform tasks related to maintaining the integrity of patient care standards and efficient unit operations.
1)Demonstrates ability to respond to emergency situations
2)Provides a safe environment for patients such as correct use and documentation of restraints and restraint alternatives, proper use of equipment coming in direct contact with patients
3)Maintains and develops technical skills by attending mandatory unit inservices, participates in 5.4 Continuous Quality Improvement Process.
4)Assists with maintenance of equipment for the unit
5)Works within the policies and procedures of infection control by demonstrating knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and proper attire when in direct contact with patients on isolation precautions.
6)Maintains confidentiality of information.
7)Demonstrates responsibility and accountability for conduct and attire while on duty.
Performs cleaning tasks.
1)Cleans and maintains patient rooms and other patient/staff areas.
2)Cleans unit specific equipment/instruments as needed, and prepares all instruments for sterilization
3)Uses cleaning materials safely appropriately in accordance with MSDS sheets.
4)Utilizes proper body mechanics when performing housekeeping duties.
Processes therapeutic and diagnostic orders.
1)Transcribes orders in an accurate and timely manner.
2)Notifies patient care team and ancillary departments of outstanding orders.
3)Determines priority and schedules patient procedures/tests in an appropriate sequence.
4)Maintains current list of attending and consulting physicians.
5)Communicates pertinent and timely information to appropriate departments and staff
Ensure accurate maintenance of the patient's medical record.
1)Verifies all documents contained within the medical record for complete and accurate patient demographic information.
2)Assembles the medical record in the proper sequence checking that all forms are correctly stamped or labeled
3)Checks all charts at the end of each shift to ensure all orders are transcribed
4)Maintains patient medication records, dietary and care kardexes as assigned. Maintains unit logs and checklist.
5)Assembles admission, transfer, and discharge charts in proper order. Strips charts as needed
Computer skills
1)Enters all required data into computer, such as diets, transfers, discharges, update on nurse revision screens, etc
2)Processes patient charges for services.
3)Maintains accurate data base, i.e., clinical data screens, consulting MD's, allergies

Physical Requirements and Working Conditions: Ability to rotate shifts to provide coverage -Exposure to various contagious disease, radiation, chemicals, caustic materials, electrical equipment and hazards associated with their use. -Demands which require walking, standing, bending and lifting throughout the day. -Ability to lift 50lbs. or more may be required. -registry rotates shift
