EDUCATION:
Master of Science Degree in Nursing required.
EXPERIENCE/TRAINING:
Minimum of 2 yearsโ experience as a Registered Nurse or Nurse Practitioner; outpatient setting preferred; experience with EHR preferred.
LICENSES/CERTIFICATIONS:
Required:
Current unrestricted Registered Nurse (RN) license issued by California Board of Registered Nursing (BRN)
Current unrestricted Nurse Practitioner (NP) with Furnishing License issued by California Board of Registered Nursing (BRN)
Current National Provider Number (NPI) issued by Center for Medicare and Medicaid Services (CMS)
Current Drug Enforcement Administration (DEA) number issued by DEA
Current Basic Life Support Provider (BLS) issued by American Heart Association
Preferred:
Current Certification in Occupational Health Nursing (COHN-S)
SKILLS:
The Nurse Practitioner (NP) coordinates and case manages the patient through the full continuum of care, orchestrating patient flow through a comprehensive care plan, working to ensure that care is appropriate, timely, authorized and conducted in the most appropriate setting. Coordinates the care of the patient serving as the liaison between community physicians and Huntington Health Physiciansโ resources. Provides clinical, professional expertise and leadership for patients, families, and staff members. The Nurse Practitioner must be able to work independently and with a team. Patient focused. Excellent customer service skills with patients, families, physicians and staff. Able to project a positive self-image and be self-motivated.
