๐Ÿ“ Description: **Job Title:** Nurse Case Manager โ€“ Skilled Nursing Facility


**Reports To:** Director of Nursing / Administrator


**Position Summary:**

The Nurse Case Manager is responsible for coordinating and overseeing the care of Veterans and Managed Care residents within the Skilled Nursing Facility (SNF). This role ensures residents receive appropriate, high\\-quality, and cost\\-effective care by facilitating communication between residents, families, the interdisciplinary team, providers, and payors. The Nurse Case Manager promotes positive outcomes, smooth transitions of care, and resident\\-centered services while maintaining compliance with facility, state, and federal regulations.


**Key Responsibilities:**


* Serve as the primary liaison between Veterans, Managed Care residents, families, and payors throughout the residentโ€™s stay.
* Coordinate admissions, care planning, and discharge planning to ensure a seamless resident experience.
* Collaborate with the interdisciplinary team (nursing, therapy, social services, dietary, activities, and medical providers) to develop individualized care plans that meet medical, functional, and psychosocial needs.
* Monitor clinical progress, therapy utilization, and care outcomes to ensure goals are met.
* Facilitate communication with Veterans Affairs, Managed Care organizations, hospitals, and other external partners.
* Provide education and support to residents and families regarding care needs, expectations, and available resources.
* Track and report outcomes, including length of stay, readmissions, and resident satisfaction.
* Ensure compliance with documentation, authorizations, and billing requirements for Veterans Affairs and Managed Care contracts.
* Advocate for residentsโ€™ needs and promote resident rights and quality of life.


**Qualifications:**


* Current Registered Nurse (RN) license in good standing.
* Minimum of 2 years of nursing experience, preferably in long\\-term care, skilled nursing, case management, or managed care.
* Knowledge of VA and Managed Care guidelines, authorizations, and care coordination processes strongly preferred.
* Strong organizational, communication, and problem\\-solving skills.
* Ability to work collaboratively with interdisciplinary teams, residents, families, and external partners.
* Compassionate, resident\\-focused approach with commitment to quality care.


**Preferred Skills:**


* Experience working with Veterans and their families.
* Familiarity with care transitions, discharge planning, and community resources.


Proficiency in electronic medical records and documentation systems.
