📝 Description: The Nurse, RN participates in the engagement, assessment and treatment planning process and as part of the program, provides hands on assistance with personal care, mobility assistance and help with housekeeping in addition to medical interventions to address needs and goals of persons served and provides direct services to people in residential settings in the areas of medical health, daily living skills, transportation, skill\\-building ,medication administration, risk mitigation and other areas based on the person centered treatment plan. Trainings at residential group homes as well as MAP training for the agency. **What you’ll do:**

* Oversees and helps residential staff coordinate medical services with primary care physicians (PCP) and specialists. Assists to obtain reports and approval of treatment plans from PCP/specialists and reports and objectively documents all changes in the condition of each individual.
* Advises supervisor and managers of any medical problems that may hinder an individual’s progress or that may be aggravated by activities/daily routines.
* Conducts/oversees audits of medical documentation to ensure compliance with all regulatory requirements.
* Monitors and assists C.N.A staff with supervisions and assists program staff with coordination of medical needs with health care professionals, including inpatient services or when an individual is receiving health care from an outside source.
* Participate in all required clinical and skill trainings including Welle or approved Agency Safety trainings.
* Adherence to Agency Safety and Risk Mitigation policies and protocols.
* Reviews seizure and medical incident reports and makes recommendations to program staff and assist with any follow up with participant’s medical practitioners if necessary
* Assist persons with various tasks including movement, exercise, showers, laundry, transporting from bed to wheelchair, wheelchair to shower, housekeeping and upkeep of client living areas.
* Participate in managers’, clinical and staff meetings (P and P) on designated rotation to review health care needs and global health care planning with all staff.
* Oversees/provides remote or direct care support on the handling of medical emergencies as needed. Ensures that medical emergencies are properly reported. Hands on support, changing dressings, bandages, and other medical interventions within the nurses scope of practice.
* Ensures that human rights, safety and respect for Individuals are safeguarded in all interventions and interactions. Maintains documentation on emergency events.
* Conduct MAP audits at each residential sites at least yearly. If in a direct care setting, medication and money audits twice a month.
* All other duties assigned by supervisor.

**What you’ll need:*** **Education/Certification:**
+ Registered Nurse with current Massachusetts license to practice required.
* **Experience:**
+ Minimum of 1\\-year experience working with similar population in a human services environment required.
+ Knowledge of DMH regulations. Knowledge of DS/ABI preferred.
