**The Position**In this position, you’ll support healthcare providers by delivering high\\-quality, tech\\-enabled care to patients managing chronic conditions. Our Client is looking for someone passionate about patient education, driven to handle a high volume of triage calls, and flexible in a fast\\-paced, evolving healthcare environment.

**Duties \\& Responsibilities**
* Enroll patients into the Remote Patient Monitoring (RPM) program
* Educate and train patients on proper device usage and expectations
* Monitor daily physiological data captured from patient devices
* Conduct regular coaching calls to encourage patient compliance
* Follow up with patients who are non\\-compliant with data submissions
* Serve as the primary point of contact for patients to build rapport and maintain satisfaction
* Analyze incoming patient data and triage abnormal readings promptly
* Escalate urgent health concerns to providers for further evaluation
* Provide health coaching for high\\-risk patients with diabetes, hypertension, and CHF
* Document patient assessments, health education, and planned interventions in electronic health records
* Maintain detailed and accurate notes of all patient interactions and communications
* Support providers by presenting relevant patient information for clinical decision\\-making
* Collaborate with providers and practice teams to ensure seamless care coordination
* Identify and professionally address provider or workflow challenges to ensure RPM program success
* Adapt workflows and procedures based on provider or practice feedback
* Communicate effectively with providers regarding patient needs and necessary care adjustments



**Supervisory Responsibilities**
* No immediate direct reports



**Skills \\& Qualifications**
* LPN or RN required
* Minimum of 5 years work experience working in practice or healthcare setting
* Must be fluent in both English and Spanish
* Strong verbal communication and patient education skills
* Ability to build patient trust and encourage program compliance
* Experience in health coaching, especially for chronic conditions like diabetes, hypertension, and CHF
* Detail\\-oriented with strong documentation and EHR charting skills
* Ability to analyze and respond to clinical data and alerts
* Knowledge of Remote Patient Monitoring (RPM) systems and protocols
* Familiarity with care coordination and healthcare team communication
* Skilled in addressing patient concerns with empathy and professionalism
* Able to identify practice challenges and suggest workflow improvements
* Comfortable working in a fast\\-paced, patient\\-facing healthcare environment
* Adaptable and responsive to evolving provider and organizational needs



**Work Environment**
* Part\\-time Remote role
* Alternating weekends, 8am\\-5pm in time zone of residence
* Residency in one of the following states will be accepted: VA, GA, TX, AZ, MD, PA, NY, or SC.



**Compensation**
* Position offers an hourly rate of $26\\-$28, with additional benefits



**Certificates, Licenses, Registrations**
* Registered LPN, LVN or RN
* Active compact nursing license (NLC)
