**The Position**In this position, you’ll support healthcare providers by delivering high\\-quality, tech\\-enabled care to patients managing chronic conditions. Our Client is looking for someone passionate about patient education, driven to handle a high volume of triage calls, and flexible in a fast\\-paced, evolving healthcare environment.

**Duties \\& Responsibilities**
* Monitor daily capture of patient physiologic data from devices
* Analyze collected data and prioritize outreach based on highest clinical needs
* Perform patient outreach to triage out of range readings in a timely manner
* Escalate cases which require provider attention
* Document assessment data, education provided, and lifestyle interventions planned in patient charts
* Capture detailed and concise notes of all patient interactions
* Act as a resource to patients in enabling access to the provider and presenting information to the provider for medical decision\\-making



**Supervisory Responsibilities**
* No immediate direct reports



**Skills \\& Qualifications**
* LPN or RN required
* Minimum of 5 years work experience working in practice or healthcare setting
* Strong verbal communication and patient education skills
* Ability to build patient trust and encourage program compliance
* Possess ability to change workflow and processes as requested by provider or practice
* Ability to analyze and respond to clinical data and alerts
* Knowledge of Remote Patient Monitoring (RPM) systems and protocols
* Familiarity with care coordination and healthcare team communication
* Skilled in addressing patient concerns with empathy and professionalism
* Able to identify practice challenges and suggest workflow improvements
* Comfortable working in a fast\\-paced, patient\\-facing healthcare environment
* Adaptable and responsive to evolving provider and organizational needs



**Work Environment**
* Part\\-time Remote role
* Residency in one of the following states will be accepted: AZ, GA, MD, NY, PA, SC, TX, or VA.



**Compensation**
* Position offers an hourly rate of $22\\-$28, with additional benefits



**Certificates, Licenses, Registrations**
* Registered LPN, LVN or RN
* Active compact nursing license (NLC)
