EDUCATION/EXPERIENCE: Graduation from accredited school of nursing. BSN preferred. Two years' of acute care experience required. Prefer acute care experience within the last 10 years.
LICENSURE, CERTIFICATION, and/or REGISTRATION: RN licensure in applicable state. Basic Cardiac Life Support (BCLS) required. Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS) or Pediatric Emergency Assessment, Recognition and Stabilization (PEARS) may be required within one year in select areas.
ESSENTIAL FUNCTIONS:
1\\. Provides patient/family centered care, acting as partner and adviser, assists and supports patients and families to manage their health care optimally, respecting culture and values, individual needs, health goals and treatment preferences.
2\\. Directs and leads other assigned team members and collaborates with multidisciplinary team members to provide age/developmentally appropriate care in accordance with nursing standards of care and practice.
3\\. Applies critical reasoning and astute clinical judgment in order to expedite appropriate care and treatment, especially given that the patient may present complex problems or potentially life threatening conditions.
4\\. Consults with provider team as needed when dealing with complicated patients. Communicates vital patient information to providers.
5\\. Facilitates increases in efficiency and throughput of various clinics by reviewing medical information including medications and placement of orders. Gathers past medical history and other pertinent patient assessments to further increase patient experience prior to the first visit.
6\\. Facilitates the continuity of care using the nursing process, multidisciplinary collaboration, and coordination of appropriate health care services and community resources across the care continuum. Interacts with patients during face\\-to\\-face encounters or through a variety of telecommunication strategies, often establishing long\\-term relationships.
7\\. Provides nurse telephone triage service to patients serving as a nursing expert of specialized patient care, assessment, recognizing and interpreting signs and symptoms of immediate distress, documentation, treatment intervention and patient/family education and support in accordance with education/experience, established policies and procedures, and state licensure requirements.
8\\. Applies the provisions of the American Nurses Association Code of Ethics for Nurses to professional obligations and for the patients entrusted to their care. Participates in professional development by maintaining required competencies, identifying learning needs and seeking appropriate assistance or educational offerings. Demonstrates the ability to function in a professional setting through active participation in a professional practice model. Includes participating in shared governance, adherence to standards of care, participation in peer review and professional development promoting leadership and clinical excellence, effective team skills, autonomous practice and acknowledgment of accountability for actions and critical thinking. Pursues lifelong learning that updates and expands clinical, organizational, and professional roles and responsibilities.
9\\. Focuses on patient safety and the quality of nursing care by applying appropriate nursing interventions, such as identifying and clarifying patient needs, performing procedures, conducting health education, promoting patient advocacy, coordinating nursing and other health services, assisting the patient to navigate the health care system, and evaluating patient outcomes.
SKILLS/QUALIFICATIONS:
* Ability to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership and others
* Ability to assess nursing needs of acute and chronically ill patients and their families Independently seek out resources and work collaboratively
* Ability to establish and maintain effective working relationships
* Ability to teach patients and families in accordance with the nursing plan of care
* Good computer skills
* Documents and communicate pertinent information using computer and/or paper documentation tools
* Prolonged standing and walking with the ability to move or lift at least fifty pounds
* Working knowledge of infection control procedures and safety precautions
* Working knowledge and completion of appropriate TJC and other regulatory requirements
