Presbyterian Village North is currently hiring a Full Time **MDS Coordinator RN** to join our **Skilled Nursing Department.**

RN license required; Full Time \\- Monday \\- Friday (8:30am \\- 5:00pm flexible to business needs) \\+ 7 day on\\-call rotation every 4 weeks.

**ESSENTIAL DUTIES:**


Models a service and solutions oriented attitude towards others, takes ownership in solving problems and takes the initiative to make things better, is friendly and courteous to residents, patients and co\\-workers



Ensure resident/patient, visitor, and co\\-worker safety by remaining aware of and reporting any new or existing safety hazards



Knowledge of Medicare pre\\-admissions for qualifying diagnosis and coverage eligibility



Obtains/ maintains physician certificates for each Medicare resident



Maintain good attendance record



Maintains MDS schedule



Completes MDS 3\\.0 for Medicare residents



Assures that all MDS information is completed and transmitted per policy, and is compliant with all regulations and forwarded to billing office


Tracks Medicare resident benefits days, validates daily Medicare census and coordinates this information with accounting representative and therapy representative, to ensure accurate billing (if applicable)



Assures all quarterly resident assessments are completed



Assures all care plans completed and updated as needed



Coordinates daily PPS meetings and weekly Medicare meetings



Reviews nursing documentation of residents and provides guidance/training as needed, to ensure charting is appropriate and meets coverage guidelines



Responsible for completing case management to ensure adequate documentation for reimbursement



Coordinates discharge of residents



Works with Clinical Liaison to verify appropriateness of admission and admission diagnosis codes



Maintains weekly communication with insurance case manager to receive approval for resident treatment and communicate with physician on insurance resident admission for timeliness of physician visits



Communicates with rehab department on routine basis, to discuss therapy services



Communicates with Life Enrichment and Social Services to coordinate assessment in a timely manner



Recruit, hire, train, coach, and evaluate departmental staff (management staff only)



Monitor residents for changes in condition, ensure MD, and family, are notified and appropriate documentation is reflected in the medical record



Assess residents initially and annually and complete resident service plans on an annual or as needed, basis



Maintains the Residents Roster and Acuity Levels within the department (i.e., State Boards, QA monitor)



Review and follow\\-up all consultant reports in a timely manner



Reviews monthly physicians orders and ensures accuracy



Monitors psychotropic behavior sheets and makes recommendations as needed



Complete performance evaluations of all licensed nurses, mediation aides and CNAs



Notify Supervisor/Administrator of incident reports concerning abuse



Thoroughly investigate any complaints and concerns and report to Administrator



Attend all assigned committee meetings



Review incident report documentation and ensures follow\\-up according to facility policy



Make rounds to verify clinical compliance and quality of care and provide coaching and disciplinary action as indicated within the facilities policies and procedures



Ensures all Life Enrichment programs/QA monitors are implemented on assigned units



Perform charge nurse duties, in cases of staffing emergencies, as required to meet the residents immediate needs


Responsible for on\\-call staffing every 4th week rotation (24 hours Friday\\-Thursday); includes $250\\.00 incentive pay





Other duties as assigned by Director of Nursing

**MINIMUM QUALIFICATIONS:**



Valid Texas RN license



AANAC RAC\\-CT Certification a plus



In depth knowledge and demonstration of MDS skills



Minimum of 2 years experience with RAI process/MDS/PPS/Medicaid CMI in a Medicare/Long Term Care setting



Experience with electronic medical charting



Knowledge of SNF, Medicare documentation and reimbursement guidelines



Demonstrated computer competencies



Knowledge of rehabilitation theory and application



Knowledge of current professional nursing theory and practice



Ability to read, write and effectively communicate in English



Excellent verbal and written communication skills



Knowledge of restorative nursing care practices



Demonstrated supervisory and managerial skills



Ability to maintain a stable disposition in performing work in association with staff and residents



Ability to maintain a high level of performance in an environment of frequent demands and changes



Ability to perform Essential Duties


Ability to work a 7day on\\-call rotation every 4 weeks





Genuine caring for and interest in the care of mature adults, persons with disabilities and hospice patients



Ability to perform work tasks within the Physical Demand Requirements



Ability to comply with the Resident Bill of Rights (LTC) and Employee Responsibilities
