**Salary:**

The pay range for this position is $26\\.00 – $29\\.00 per hour, based on a combination of licensure, education, and relevant experience. Candidates with strong clinical skills and prior experience in behavioral health may qualify for compensation at the higher end of the range.

**Role Summary:**
We’re looking for a compassionate and detail\\-oriented Licensed Practical Nurse (LPN) to provide direct medical care and support to clients facing substance use, mental health, or co\\-occurring challenges.
In this role, the LPN will deliver high\\-quality care by administering medications, monitoring detox protocols, completing documentation, and assisting with lab processing. The LPN will also provide education to clients regarding prescribed medications and support their recovery journey with compassion and clinical accuracy. This position operates under the supervision of the Director of Nursing or a Registered Nurse, ensuring timely execution of medical orders and clear communication of client needs.

**Responsibilities:**
* Complete nursing intake assessments, triage, and initiate detox treatment plans under the supervision of the Director of Nursing or RN.
* Conduct daily nurse visits and monitor client health status, documenting any changes in condition or behavior.
* Administer medications, provide vaccinations, and deliver necessary treatments while onsite.
* Maintain complete, accurate, and compliant medical documentation in accordance with HIPAA, facility policies, and state/federal regulations.
* Initiate and manage required clinical and legal documentation related to client care and treatment.
* Collaborate with providers, RNs, LPNs, and other medical professionals to coordinate client healthcare and ensure continuity of care.
* Communicate with clients and their families to address questions, concerns, or complaints with professionalism and compassion.
* Provide emotional and psychological support to clients and families during care transitions or moments of distress.
* Respond to medical emergencies such as injuries, overdoses, or cardiac events, and initiate appropriate actions per protocol.
* Coordinate with hospitals during client transfers, including nurse\\-to\\-nurse communication and receiving discharge paperwork upon client return.
* Monitor and maintain medical supply inventory, and place orders as directed by the Director of Nursing or Charge Nurse.
* Attend and participate in staff meetings and trainings.
* Perform other duties as assigned by the Director of Nursing to support overall nursing operations.
**Preferred Qualifications:**
* Education and/or experience related to substance use, mental health, or co\\-occurring disorders, including understanding of chemical dependency and behavioral health needs.
* Experience with crisis management, including de\\-escalation and emergency response.
* Knowledge of HIPAA regulations and commitment to maintaining confidentiality.
* Familiarity with COWS (Clinical Opiate Withdrawal Scale) and CIWA (Clinical Institute Withdrawal Assessment) protocols.
* Strong interpersonal and communication skills, with the ability to work both independently and collaboratively within a multidisciplinary team.
* Effective time management, problem\\-solving skills, and the ability to maintain professional boundaries in a clinical setting.
* Sensitivity and responsiveness to religious, cultural, and individual differences among clients and colleagues.
**Minimum Requirements:**
* Valid Practical Nursing License (LPN) in the State of Illinois
* Valid Driver’s License
* Current CPR and First Aid Certification or willing to obtain.
* Must successfully complete Background Check, Drug Screen, and TB Test
**Physical Requirements:**
* Light lifting, light walking, able to walk up and down stairs, and extended periods of time sitting.
