📝 Description: Job Summary:



The LVN provides direct nursing care to medically fragile residents in a CLHF setting. Under the supervision of an RN or physician, the LVN ensures high\\-quality, compassionate care tailored to each patient’s clinical needs.



Key Responsibilities:



* Administer prescribed medications and treatments accurately.
* Provide skilled care for residents with tracheostomy, ventilator dependence, G\\-tubes, and wound care needs.
* Monitor and record vital signs, intake/output, and patient status changes.
* Communicate with an interdisciplinary team regarding resident conditions and care plans.
* Document all care provided in accordance with state regulations and facility policies.
* Respond promptly to patient needs and emergencies.
* Assist with ADLs (activities of daily living) when necessary in coordination with CNAs.

Support infection control and safety protocols.

Requirements:



* Current LVN license in the state of California.
* Valid BLS/CPR certification.
* Previous experience in a subacute, CLHF, or skilled nursing environment preferred.
* Familiarity with ventilator and trach care is highly desirable.
* Strong documentation and communication skills.
* Ability to work independently and collaboratively in a small team setting.

Physical ability to perform nursing duties, including lifting, bending, and long periods of standing.

* Shifts: 12\\-hour shifts, the rate is based on competitive compensation, depending on experience.
