**Job Summary**

* Provide direct patient care as initiated by the registered nurse
* Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care
* Report to supervisor on all patients under your care.
* Provide accurate and timely documentation
* Participates in coordination of home health services with team
* Performs other duties as assigned by the registered nurse

**Qualifications**

* Currently a Licensed Vocational Nurse (LVN) in state of employment
* Previous experience as a Licensed Vocational Nurse is preferred
* Excellent interpersonal and client care skills
* Valid driver’s license
* First aid/CPR certification required
