This position is responsible for providing care to assigned patients that meets their physical, psychological, social, cultural, spiritual, and developmental needs. Functions as a contributing member of the health care team.


**Responsibilities:**


* Assists the team leader in assessing the status of patients with simple health problems on admissions and on a continuing basis reporting changes in health status to the RN and / or Physician.
* Contributes information to the plan of care.
* Implements the plan of care, under the direction of the RN and / or Physician and within the scope of practice, to meet the basic needs of assigned patients and families.
* Documents data for assessments, interventions and effectiveness of care provided in the patients' medical record. Findings are reported to the RN and / or Physician.
* Participates in patient education activities as directed by the RN and / or Physician.
* Assists in the preparation of the patient and family for discharge under the direction of the RN and / or Physician.
* Administers medications to assigned patients in accordance with clinical specific standards.
* Responds to emergencies.
* Identifies safety hazards and reports them.
* Demonstrates competence to perform assigned patient care responsibilities in a manner that meets the age specific and developmental needs of patients served by the department.
* Appropriately adapts assigned patient assessment, treatment, and/or care methods to accommodate the unique physical, psychosocial, cultural, spiritual, age specific and other developmental needs of each patient served.
* Demonstrates adherence to the CORE values of Santa Rosa Health Care.
* Perform other duties as assigned.


**Requirements:**


**Education**


* High School Diploma or GED Required Graduate from an accredited school of vocational nursing


**Experience**


* Experience in clinical setting preferred


**Licenses, Registrations, or Certifications**


* Licensed as a LVN in Texas BLS Certification required
