📝 Description: Nurses needed to visit patients: **Private Duty**
**Responsibilities:**
* Provide nursing care to elderly, Children and/or disabled patients
* Assess patient needs and develop corresponding treatment plans
* Monitor patient status and notify office staff and supervising physician of significant changes
* Communicate with supervising physicians and other members of the care team to provide effective care
* Educate patients, family members, and caregivers on their conditions, diagnoses, medications, and treatments
* Ensure patient records are accurate and complete
* Serve as a patient advocate and provide advice and support to the patient and his/her family
* Record a patient’s medical history accurately
* Monitor patients’ condition including fluid intake and output and compose patient charts
* Provide emotional and psychological support when needed
* Communicate with patients’ family or friends to provide advice, comfort and release instructions
**Qualifications:**
* Previous nursing experience or Entry\\-level
* Strong communication skills and ability to teach
* Must be trustworthy, friendly, and dependable
* Strong understanding of nursing, medical skills, and care plans
* Must be self\\-motivated and able to work well independently
* MUST HAVE A VALID ILLINOIS LICENSE FOR LPN
* Proficiency in Spanish Language an advantage
**Skills**
* Proven experience as LPN or similar role or Entry\\-level
* Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
* Understanding of confidentiality obligations and nursing best practices
* Excellent knowledge of medical and hospital terminology
* Good physical condition and stamina
* Excellent communication and interpersonal skills
* Compassionate and able to handle stress
* Successful completion of LPN licenses program is a must
* Vent experience
