📝 Description: **Job Summary:**


The Licensed Practical Nurse (LPN) is responsible for providing professional nursing care to the agency’s home care clients.


**Responsibilities:**


* Ensure timely and proper completion of all agency related documents and ensure that all documentation meets NYDOH regulations and agency standards.
* Provides supervision of HCSS, PCA, and HHA staff to ensure safety and compliance with plan of care.
* Teach, train, and provide oversight to HCSS, PCA, and HHA staff – including proper use of of durable medical equipment other care aides.
* Complete routine home visits.
* Prefill medication for clients.
* Perform agency requested “Meet \\& Greets” to ensure appropriateness and ability to provide a safe plan of care in the home environment.
* Perform client’s personal care and/or assist with activities of daily living as needed.
* Perform other duties as assigned.

**Pay:** $22 \\- $25 per hour


**Position Requirements:**


* Graduate of an accredited School of Nursing
* Valid and current New York State Licensed Practical Nurse license and registration.
* Previous home care experience preferred.
* Ability to successfully complete a Competency \\& Proficiency Test and Skills Evaluation appropriate to job title before hire.
* Excellent verbal and written communication skills.
* Excellent human relations and customer service skills.
* Ability to function with minimum direct supervision, to make decisions, to problem solve, and to manage emergencies.
* Ability to relate to diverse and multi\\-cultural populations.
* Appearance is professional and complies with agency dress code.
* Ability to lift up to 25 pounds and to sit, stand, bend, stoop, kneel, pull, climb, reach, and perform repetitive movements of the upper extremities.
* Pass a physical examination and immunization requirements after being offered employment and complete a health assessment annually, thereafter.
* Possess a current, valid New York State driver’s license and a reliable vehicle.
* Completion of CCOR's orientation program.
