Major Responsibilities:
Maintains staff schedules and schedules patients for admission.
1)Maintains schedules for all Liaisons and Admission nurses in the field. Assigns new admissions to Admission nurses with assistance from the Intake coordinator.
2)Contacts patient/family to set up initial appointment for presentation for hospice services in the home setting. Updates intake data collection tool appropriately.
3)Responsible for coordination of patient information. Insures compliance with HIPPA guidelines. Process admission documentation, establishes medical record.
4)Enters patient information including referrals, admissions, insurance demographics into pre-admission data base . Responsible for maintenance of patient’s medical records with established policies and procedures.
5)Captures referrals and important referral information.
6)Telephonic Customer Service with hospice referral sources
Responsible for verify summary of service documents for the purpose of accurate billing and payroll.
1)Ensures there is a visit report for every visit listed and follows SOP compliance guidelines
2)Contact clinical staff for any discrepancies and inform Clinical Manager/Manager of Business Services when information is not received as needed.
3)Verifies activity codes, patient ID numbers, visit dates and times.
4)Validates accuracy of visit entry for payroll/billing process.
5)Assists with maintaining patient files for processed time documents and other payroll documentation
Responsible for support function and timeliness of execution to meet the required regulations.
1)Data entry of all new referrals into the computer system and assignment of patient ID numbers.
2)Copy and distribute new referrals, update active referral list update pending referral list and update discharge referral list.
Education/Experience Required:
High School education 3-5 years prior experience in a medical field such as a hospital or medical office. • Good communication skills
A valid driver’s license issued by the Division of Motor Vehicles.
Physical Requirements and Working Conditions:
This job requires reaching, bending, stooping, lifting. Ability to work in a fast-paced, changeable environment. Must be willing to work occasional overtime if needed.
If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.

Pay Range
$21.45 - $32.20
