Hospice Licensed Vocational Nurse (LVN) - Per Diem at Next Care Hospice

📝 Description: We are seeking a full\\-time LVN Licensed Vocational Nurse to join our team! You will be responsible for delivering high\\-quality care to assigned Hospice patients. Clinical experience is preferred and candidates with Hospice experience are highly desirable.





**Responsibilities:**



* Provide Skilled Nursing Care in a home or facility setting
* Ensure the appropriate use of meds, DME, and supplies
* Educate and support patients, families, and caregivers
* Participate in IDT and updating the Plan of Care
* Provide our patients and families the best care possible
* Daily travel is required when working

**Qualifications:**



* Valid LVN License in California
* One year or more of experience in either Home Health or Hospice is preferred, but not required
* Previous experience in a hospice setting is a plus
* Ability to build rapport with patients
* Compassionate and caring demeanor
* Excellent written and verbal communication skills
