Position Summary

The Health Center RN Team Lead provides clinical leadership and daily operational oversight of Health Center Registered Nurses (HCRNs) within a Federally Qualified Health Center (FQHC) setting. This role ensures the consistent delivery of high\\-quality, trauma\\-informed, and patient\\-centered nursing care across ambulatory workflows. In close partnership with the Manger of Health enter Nursing, providers, and interdisciplinary tams, the RN Team Lead drives clinical excellence, supports regulatory compliance, and leads efforts to close care quality gaps. This position also delivers direct nursing care and patient education as needed to support optimal outcomes.



Core Job Responsibilities

* Leads daily clinical workflows for HCRNs including triage, chronic disease management, care gap closures, medication refills, and prior authorizations to ensure timely and effective patient care.
* Serves as the clinical liaison between HCRNs, providers, and nursing managers to resolve workflow challenges and support cross\\-functional collaboration.
* Serves as a resource and mentor to HRCNs, providing onboarding support, skills assessments, and guidance on clinical best practices.
* Ensures adherence to clinical protocols, infection control policies, and scope of practice standards while promoting consistent, quality\\-driven care.
* Supports HRCN compliance with documentation and care delivery standards, including timely and accurate charting in Epic (e.g., lab results, medication refills, patient encounters, communications).
* Monitors clinical KPIs and partners with ACCESS leadership to implement quality improvement initiatives that elevate patient outcomes and operational efficiency.
* Participates in audits and readiness efforts for accreditation, licensing, and compliance reviews.
* Collaborate with nursing managers to identify staffing training needs and facilitate continuous professional development and performance feedback.
* Provides direct nursing care within HCRN scope of practice, ensuring patient\\-centered, trauma\\-informed care, and culturally competent service delivery.
* Supports interdisciplinary collaboration to ensure coordinated, continuous patient care; contributes to clinical meetings and assumes leadership responsibilities in the managerโ€™s absence, including performing other assigned duties that advance health equity initiatives.


Requirements/Preferences

* Active and current, unrestricted Registered Nurse (RN) license in the State of Illinois.
* Associateโ€™s degree required; Bachelor of Science in Nursing (BSN) preferred.
* Minimum of three (3\\) years of clinical nursing experience, ideally within a community health or ambulatory care setting.
* Proven leadership skills with experience mentoring, training, or supervising clinical staff.
* Strong proficiency in Epic or comparable electronic health (EHR) system.
* Preferred experience in a Federally Qualified Health Center (FQHC) or working with underserved populations.
* Bilingual (English/Spanish) preferred or ability to effectively engage with patient using interpretation services.


Working Conditions/Equipment

* Normal office environment is regularly required and health center environment occasionally required.
* Local travel between health centers and community events is required. Modes of transportation must be available.
* If a personal vehicle is used, a valid driver's license and proof of insurance are required.
* For remote or hybrid work, a suitable and secure working environment, acceptable internet speed, and adequate childcare arrangements are required.

The pay ranges provided represent the minimum to mid\\-range for positions. Actual compensation will be determined based on a combination of factors including years of experience, educational background, market conditions, and available grant funding.
