We are seeking a caring and reliable Full\\-Time Licensed Practical Nurse (LPN) to join our home health care team. As an LPN, you will provide compassionate, patient\\-centered care to individuals in their homes under the supervision of a Registered Nurse (RN). You will play a critical role in helping patients manage their health, recover from illness or surgery, and improve their quality of life while staying in a familiar and comfortable environment.
**Key Responsibilities:**
* Deliver direct patient care, including monitoring vital signs, wound care, and administering medications.
* Follow established care plans under the supervision of an RN or physician.
* Assist patients with daily living activities, including mobility, hygiene, and nutrition.
* Educate patients and their families about home care practices, medication administration, and health management.
* Collaborate with healthcare providers and the home health team to ensure the best possible patient outcomes.
* Document patient progress, health status, and care provided in the electronic medical record (EMR) system.
* Monitor for any changes in a patient’s condition and promptly report to the RN or physician.
* Adhere to agency policies, procedures, and state/federal regulations to ensure compliance and quality care.
**Qualifications:**
* Active and unrestricted Licensed Practical Nurse (LPN) license in Florida.
* Minimum of 1 year of clinical experience, preferably in home health or a similar setting.
* Strong knowledge of home health care practices and state regulations.
* Proficiency in electronic medical record (EMR) systems.
* Excellent communication and interpersonal skills.
* Ability to work independently, manage multiple patients, and prioritize tasks effectively.
* Compassionate, patient\\-focused, and detail\\-oriented.
* Valid driver’s license and reliable transportation.
