The Opportunity
Weâre hiring a **Clinical Liaison (RN)** to support the launch and growth of a new hospital\\-partnered service line in **Philadelphia**.
Youâll act as the face of myLaurel inside the hospitalâbuilding trust, identifying eligible patients, and promoting our home\\-based acute care programs. This is a hybrid clinical\\-strategic role perfect for a nurse who excels in care coordination, patient education, and stakeholder engagement.
Key Responsibilities* **Hospital Engagement** â Build strong relationships with care teams, physicians, and hospital leadership
* **Referral Management** â Identify eligible patients and guide them into myLaurelâs acute care programs
* **Patient Education** â Clearly explain services and care plans to patients and caregivers
* + ď¸ **Operational Integration** â Seamlessly embed myLaurel into hospital discharge and care workflows
* **Strategic Growth** â Collaborate with internal teams to drive program adoption
* **Track Results** â Monitor key performance metrics including referrals, patient satisfaction, and outcomes
* **Cross\\-Team Collaboration** â Coordinate with myLaurelâs internal care and operations teams
A Day in the Life* Attend hospital huddles and rounds
* Conduct chart reviews and referral assessments
* Deliver program education to patients, caregivers, and staff
* Track referral data and outcomes
* Serve as a clinical ambassador for home\\-based acute\\-level care
â What Youâll Need* **Active, unrestricted RN license (required)**
* **5\\+ years of clinical experience** â **ICU or ER experience strongly preferred**
* Expertise in care transitions, discharge planning, and readmission prevention
* Strong communication and relationship\\-building skills
* Proficiency with EHRs and hospital systems
* BSN or related degree preferred
* Bonus: Experience in program development or liaison roles
* ď¸ **Note for Candidates:** ICU or ER experience is highly preferred. Candidates without this background will be reviewed on a case\\-by\\-case basis.
