The role you'll contribute: Responsible for working collaboratively with specific patient populations to promote optimal clinical and resource utilization allocation. This position coordinates care across the health care continuum within the physician office setting. This role ensures safety, best practices, and high\\-quality standards of care are maintained. In addition, this position analyzes current systems and variances to identify opportunities for improvement. Additionally, this role promotes quality of care through collaboration with service team members, patients, and families. Responsibilities for this role include coordinating self\\-management support and disease registry activities for the office's patient population.
**The value you'll bring to the team:**
* Manages specific patient populations across the continuum, focusing on complex, high\\-risk, high\\-cost, and multi\\-diagnosis patients.
* Oversees the disease registry database, ensuring it is up to date and arranging follow\\-up services for patients overdue for visits, labs, or referrals.
* Identifies patients not meeting clinical goals and arrange follow\\-up services by protocol or as appropriate.
* Creates patient, physician, and office\\-level quality performance reports.
* Conducts pre\\-visit chart reviews, identifying needed preventive health maintenance, immunizations, and chronic disease interventions.
Qualifications
**The expertise and experience you'll need to succeed:**
* 3 Year of relatable Work Experience Required
* Driver's License (DL) Current \\& valid Required
* Bachelor's of Nursing Preferred
