**Job Summary:**
We are seeking a skilled and compassionate Registered Nurse (RN) to join our team to do Oasis assessments and Visits , providing direct patient care, assess patient health needs, and develop care plans in coordination with physicians and other healthcare professionals.
**Key Responsibilities:**
* Conduct initial and follow\\-up patient assessments in the home setting.
* Develop, implement, and revise individualized care plans.
* Administer medications and treatments as prescribed.
* Educate patients and their families about their care plans, medications, and lifestyle adjustments.
* Monitor and document patient progress in accordance with agency policies and regulatory guidelines.
* Coordinate with physicians, therapists, and other healthcare professionals to ensure optimal patient care.
* Adhere to infection control and safety protocols.
* Participate in on\\-call rotations as required.
**Qualifications:**
* Active Registered Nurse (RN) license in Florida.
* Minimum of 2 years of nursing experience (home health experience preferred).
* Current CPR certification.
* Valid driver’s license and reliable transportation.
* Strong organizational and communication skills.
* Ability to work independently and make sound clinical decisions.
