๐Ÿ“ Description: #### **SUMMARY:**


The Assistant Director of Health Services supports the Health Services Director in overseeing and coordinating the delivery of high\\-quality health care and custodial services for all residents. This role is responsible for ensuring compliance with state regulations, fostering a culture of person\\-centered care, and providing leadership and supervision to the Health Services Department staff. The Assistant Director also serves as a clinical resource, provides staff training, and assumes on\\-call responsibilities as assigned.


#### **DUTIES AND RESPONSIBILITIES**


Provide 24\\-hour on\\-call coverage for a minimum of seven (7\\) days per week on a rotating basis with the Health Services Director and/or designated nursing staff.



Conduct comprehensive resident assessments, including initial, routine re\\-assessments, change\\-in\\-condition assessments, sub\\-assessments, and self\\-medication reviews.



Develop, review, and update individualized health care plans in collaboration with residents, families, and healthcare providers.



Complete medication reviews for all residents enrolled in the medication assistance program, including controlled substance reviews.



Monitor residentsโ€™ ongoing conditions, document changes, and initiate referrals to physicians or supportive services as appropriate.



Conduct wellness checks and initiate appropriate follow\\-up care, including referrals, interventions, and documentation.



Perform monthly and quarterly clinical reviews, such as enabler safety checks and medication aide observations, ensuring compliance with policy and best practices.



Manage and maintain accurate, timely, and confidential electronic medical records (EMR) and census updates.



Facilitate resident care coordination with external providers, including physicians, pharmacies, and therapy services.



Oversee nursing documentation and ensure adherence to organizational and regulatory standards.



Support vaccination clinics, surveillance testing, and other public health initiatives.



Provide orientation, training, and ongoing education to Health Services staff to ensure competency in clinical care and regulatory compliance.



Promote teamwork, professional development, and a supportive work environment for staff.



Uphold policies, procedures, and state regulations to ensure quality care, resident safety, and operational efficiency.


#### **QUALIFICATIONS**


Current, Registered Nurse (RN) license in the State of New Jersey.



Minimum of 2โ€“3 years of nursing experience, preferably in assisted living, long\\-term care, or a similar healthcare setting.



Prior supervisory or leadership experience in a healthcare environment strongly preferred.



Strong knowledge of assisted living regulations, resident care planning, and medication management.



Proficiency in electronic medical records (EMR) systems and Microsoft Office Suite.



Excellent communication, organizational, and leadership skills.



Ability to manage multiple priorities in a fast\\-paced environment while maintaining attention to detail.



Demonstrated ability to educate, mentor, and empower staff.
